How to get a Missouri medical card

Straight answer

Get certified, then apply online. A Missouri-licensed MD, DO or nurse practitioner examines you and certifies a qualifying condition, and you apply within 30 days. It costs $28.92 for 3 years, cuts the tax to 4% instead of up to 9% plus sales tax, and lets you shop at 18.

Three steps to getting certified

Going medical still pays off with adult-use stores open, and this part of our Missouri dispensary guide walks it start to finish. Everything happens online, and nothing plastic gets mailed.

  1. A Missouri-licensed MD, DO or nurse practitioner meets with you, examines you, reviews your records and certifies a qualifying condition
  2. Apply in the state's online registry within 30 days of the certification, with a copy of your photo ID and a color photo from the last 3 months
  3. Pay $28.92. The state has 30 days to decide, and you download and print it yourself. It lasts 3 years

The list includes cancer, epilepsy, glaucoma, intractable migraines, chronic severe pain or muscle spasms, PTSD diagnosed by a psychiatrist, HIV and AIDS, terminal illness and conditions treated with habit-forming prescriptions, plus any condition the clinician judges qualifies. Past convictions don't disqualify you, and you don't have to live in Missouri.

Does it pay for itself?

Slide to what you spend in a month. In Kansas City, adult-use buys carry the 6% state and 3% local marijuana taxes plus 8.975% sales tax, 17.975% in all. Patients pay 4% plus the sales tax.

Uses Kansas City's Jackson County rates and the July 2026 to June 2027 state fee. The clinician's visit costs whatever the practice charges.

What patients get

  • Lower tax. 4% instead of the 6% state tax and the local 3% where a city has one. Sales tax still applies.
  • Bigger limits. 6 ounces every 30 days and up to a 60-day supply on hand, more if your clinician certifies it.
  • 18 and up. Patients can shop at 18, and minors with a parent as caregiver.
  • Cheaper home grow. Growing at home costs $57.83 for a patient's registration, against $115.68 for the adult-use one.
  • Every store. 226 of the 226 licensed stores we list serve patients.

Since adult use opened, the state reports patient applications fell 87% in the first program year. From November 12, 2026, state agencies can't share patient lists with the federal government without a subpoena or court order.

04

Renewals, caregivers and the gun question

Cards issued in late 2022 are hitting their first renewal now. You can renew up to 60 days early with a certification signed in the last 30 days, and an expired one stays inactive until the renewal is approved. Caregivers must be 21, can serve up to six patients, and each patient can have two.

Guns are the open question. The constitution doesn't address firearms, and the state says federal law may still bar cannabis users from owning or buying them. A January 22, 2026 federal rule narrowed "unlawful user" to recurring use, and nobody has said whether the April 2026 Schedule III move changes it for patients, so ask a lawyer. Commercial drivers still can't use it. Other states are compared in our medical marijuana card guide.
