Worth it if rec is legal?
In adult-use states you don't need a card, but the states themselves list real reasons to get one: lower taxes, bigger limits, access at 18 (here's how old you have to be to go to a dispensary). What each state says:
- Arizona: Patients 18 to 20 can register and buy medical marijuana; adult-use purchase and possession is limited to 21 and older.
- California: Patients 18 and older can buy with a physician's recommendation and have higher possession limits; holders of the state ID card (MMIC) do not pay sales and use tax on medical cannabis.
- Colorado: Medical marijuana is not subject to Colorado's 15% retail marijuana sales tax; it pays the 2.9% state sales tax instead.
- Connecticut: DCP says registered patients buy tax-free, can buy up to 5 ounces a month (vs 1.5 oz possession for non-patients), get priority entry at hybrid retailers and products with no potency caps.
- Delaware: Delaware's Division of Revenue says only buyers who show a valid medical marijuana registry card are exempt from the 15% retail marijuana tax.
- Illinois: Only registered patients (21+) may grow cannabis at home in Illinois, up to 5 plants per household; adult-use buyers cannot.
- Maine: Maine law sets a 14% sales tax on adult-use cannabis from Jan 1, 2026; cannabis sold to qualifying patients is outside that 14% category.
- Maryland: MCA sets the standard medical allowance at 120 grams of flower or 36 grams of THC per rolling 30 days, and a provider can certify more.
- Massachusetts: CCC lists exemption from sales tax on medical marijuana products, access to higher-potency products, and a 60-day supply (up to 10 oz) as patient benefits.
- Michigan: Medical sales to registered patients are not subject to Michigan's 10% adult-use excise tax (6% sales tax still applies).
- Minnesota: Registry patients don't pay Minnesota's cannabis gross receipts tax on medical purchases (Minn. Stat. 295.81).
- Missouri: Medical sales carry a 4% state marijuana tax versus 6% on adult-use, and patients can buy up to 6 oz per 30 days (adult-use possession limit is 3 oz).
- Montana: Medical marijuana is taxed at 4% vs 20% for adult-use sales (MCA 15-64-102).
- Nevada: Cannabis sold to a patient cardholder is not subject to Nevada's 10% retail excise tax.
- New Jersey: NJ-CRC says medical purchases have 0% state sales tax (municipal tax up to 2%), patients can buy up to 3 oz per 30 days versus 1 oz per adult-use transaction, and dispensaries must give patients priority service.
- New York: OCM says medical cannabis carries only a 3.15% excise tax paid by the dispensary (not added to your receipt), patients 18+ can buy at medical dispensaries, and every medical dispensary has an on-site pharmacist.
- Ohio: Medical purchases are exempt from the 10% excise tax on non-medical sales, and dispensaries may offer patient discounts.
- Oregon: OHA says cardholders may possess up to 24 ounces of usable marijuana and up to 6 mature plants at a registered grow site; a state retail-tax exemption for cardholders was not confirmed on a state page.
- Vermont: CCB lists exemption from cannabis excise tax and sales and use tax, higher possession and grow limits, delivery, and products not sold in the adult-use market.
