How to get a New York medical card

Straight answer

See a provider, get certified, shop. A New York-registered provider certifies you, in person or by video, for any condition they find clinically appropriate. You're registered automatically, there's no state fee, and a certification can last up to two years. Patients pay no 13% cannabis tax at dispensaries that serve patients.

Three steps to a card

New York runs one of the simplest medical programs in the country, and this part of our New York dispensary guide walks it start to finish.

  1. See a New York-registered provider, in person or by telehealth
  2. Get certified. You're registered with the state automatically, with no fee
  3. Bring the certification, on paper or your phone, plus a photo ID to a dispensary that serves patients

Doctors, nurse practitioners, physician assistants, dentists, podiatrists and midwives can all certify, once they've taken the state's course. There's no fixed list of conditions. The provider decides what qualifies, and since February 19, 2026, a certification can last up to two years.

What a card saves you

Slide to what you spend in a month. Adult-use buys carry 13% tax at the register, and patient buys carry none on the receipt.

The 3.15% excise on patient sales is paid by the dispensary and can't be added to your receipt, so it isn't counted here. Each dispensary sets its own patient prices, and the certifying visit costs whatever the provider charges.

What the card gets you

  • No 13% tax. Patient buys aren't taxed at the register. The dispensary pays a 3.15% excise that can't show up on your receipt.
  • A pharmacist on site. Every dispensary that serves patients has one, and stores can offer patients discounts.
  • More on hand. Since February 2026, patients can have a 60-day supply or 3 ounces and 24 grams, whichever is more.
  • 18 and up. Patients 18 to 20 can shop too, and 30 of the state's 755 licensed spots we list serve patients. Adult-use stores can't sell to patients under 21.
  • Home grow at 18. Since February 2026, patients 18+ can grow at home, and up to five caregivers can buy for you.

What to weigh first

Fewer counters is the first one. Patient pricing only applies at dispensaries registered to serve patients, and an adult-use store that sees your certification has to give you a list of the nearest ones. Out-of-state patients can buy here with their home card and photo ID, though the state's rules for it are still being written, so call first.

Guns are the open question. Federal law bars unlawful users of controlled substances from having firearms, and a January 2026 ATF rule narrowed who counts. We haven't found a New York agency that answers it for patients, so ask a lawyer if firearms matter to you. How other states compare is in our medical marijuana card guide for every state.

Quick answers

Your questions, answered

What does a New York medical card cost?

There's no state fee to become a patient. You pay only for the certifying visit, and each provider sets that price.

Can you get certified at 18 in New York?

Yes. Patients 18 and up are registered on their own once certified. Under 18, a caregiver handles the purchases.

Which conditions qualify in New York?

Any condition your provider finds clinically appropriate. The rules also name conditions like cancer, PTSD, epilepsy, MS and pain treated with cannabis instead of opioids.

Do patients pay tax at the register here?

No. The 13% adult-use tax doesn't apply, and the 3.15% excise on patient sales is paid by the dispensary and can't be added to your receipt.

Does New York honor patient cards from other states?

Yes, at dispensaries that serve patients, with proof of your home-state registration and a photo ID. The rules are still being written, so call the dispensary first.
